Wrong client versions, contact them and verify what files they use. There could be duplicated items too if previously there was a rewind. As for the rankings it's probably because GMs constantly chage their level up and down

I'd enable it from level 1 no matter what rate you have on your server. You level you get stats, simple.

Well if there's no member system in place you can't really expect to have a blocking system since there's no actual members to block. As for IP banning, it's not effective even for message boards. Your chat script is way too simple so spam is something you have to deal with manually.
